Instructions

1) First select the button below that corresponds with your child's age. Then complete the following forms, which will take about 10-15 minutes. The forms include:

• Consent to participate.

• Information form about your family and child. 

• The BASS-R screening tool with your child. 

• Feedback form.

2) After completing the forms, you and your child will have an opportunity to receive a $50 Target or Amazon gift card by participating in a 15-20 minute zoom call with a speech language therapist, scheduled at your convenience. During this zoom call your child will be asked to name simple pictures in English (i.e., house, pig, apple).

3) If you are participating with multiple children, return to the website to complete another submission.
